Vitiligo is a skin condition whose exact cause is unknown. In vitiligo, patches of skin lose their pigmentation when the pigment producing cells, the ‘melanocytes’ are attacked and destroyed. It may affect the skin, mucous membranes, eyes, inner ear or hairs leaving white patches. The usual type of vitiligo is called ‘Vitiligo Vulgaris’ (means: common vitiligo). Variant types include linear, segmental, trichrome and inflammatory vitiligo.

Medication. Topical creams that affect the immune system or control inflammation (corticosteroid) may help re-pigment the skin. These creams may be used in combination with ultraviolet light to improve results. Due to adverse reactions, these treatments are generally used for small areas on the face and neck. Those with widespread vitiligo may wish to remove pigment from the remaining unaffected areas by applying topical creams that gradually lighten the skin.

    With respect to improved repigmentation: topical corticosteroids are better than psoralen with sunlight, hydrocortisone plus laser light is better than laser light alone, gingko biloba is better than placebo, and oral mini-pulse of prednisolone (OMP) plus NB-UVB is better than OMP alone.[8]

  10. Pityriasis alba is a skin disorder commonly found in children and young adults that causes pale pink or red, scaly patches to form on the skin. When these patches clear up, the skin is left discolored, with smooth light patches taking their place. Lesions can be round, oval or irregular in shape and many patches can occur at once, especially on the face and arms. Pityriasis alba is believed to be associated with eczema and, as such, an overactive immune response is a suspected cause.

  11. Vitiligo occurs in about 1% of the United States’ and the world’s population. Vitiligo affects all races and genders equally; however, it is more visible in people with darker skin. Although vitiligo can develop in anyone at any age, it most commonly appears in people ages 10 to 30 years. Vitiligo rarely appears in the very young or very old.

  13. It is unclear what specific circumstances trigger the immune system to attack melanocytes in the skin. Research suggests that the immune system of affected individuals may react abnormally to melanocytes that are stressed by factors such as chemicals or ultraviolet radiation. In addition, the melanocytes of people with vitiligo may be more susceptible to stress than those of the general population and therefore may be more likely to be attacked by the immune system. The condition probably results from a combination of genetic and environmental factors, most of which have not been identified.

    Topical therapy: Topical steroids and topical calcineurin inhibitors are sometimes effective to restore pigment when small areas of skin are affected by vitiligo. However, in the case of topical steroids, there is risk that the skin will atrophy or sag, thin or develop stretch marks when used for too long.
