Native American Health Service Resident Rotation Seborrheic dermatitis is a superficial fungal disease of the skin, occurring in areas rich in sebaceous glands. It is thought that an association exists between Malassezia yeasts and seborrheic dermatitis. This may, in part, be due to an abnormal or inflammatory immune response to these yeasts. The azoles represent the largest class of antifungals used in the treatment of this disease to date. In addition to their antifungal properties, some azoles, including bifonazole, itraconazole, and ketoconazole, have demonstrated anti-inflammatory activity, which may be beneficial in alleviating symptoms. Other topical antifungal agents, such as the allylamines (terbinafine), benzylamines (butenafine), hydroxypyridones (ciclopirox), and immunomodulators (pimecrolimus and tacrolimus), have also been effective. In addition, recent studies have revealed that tea tree oil (Melaleuca oil), honey, and cinnamic acid have antifungal activity against Malassezia species, which may be of benefit in the treatment of seborrheic dermatitis. In cases where seborrheic dermatitis is widespread, the use of an oral therapy, such as ketoconazole, itraconazole, and terbinafine, may be preferred. Essentially, antifungal therapy reduces the number of yeasts on the skin, leading to an improvement in seborrheic dermatitis. With a wide availability of preparations, including creams, shampoos, and oral formulations, antifungal agents are safe and effective in the treatment of seborrheic dermatitis.

Vitiligo is a skin condition whose exact cause is unknown. In vitiligo, patches of skin lose their pigmentation when the pigment producing cells, the ‘melanocytes’ are attacked and destroyed. It may affect the skin, mucous membranes, eyes, inner ear or hairs leaving white patches. The usual type of vitiligo is called ‘Vitiligo Vulgaris’ (means: common vitiligo). Variant types include linear, segmental, trichrome and inflammatory vitiligo.

Medication. Topical creams that affect the immune system or control inflammation (corticosteroid) may help re-pigment the skin. These creams may be used in combination with ultraviolet light to improve results. Due to adverse reactions, these treatments are generally used for small areas on the face and neck. Those with widespread vitiligo may wish to remove pigment from the remaining unaffected areas by applying topical creams that gradually lighten the skin.

Prescription pain medication is often necessary as the pain level is very high in many people. The pain is often so intense that people cannot have any clothing touch the skin area with shingles. Drugs such as oxycodone (Oxycontin, Roxicodone), morphine, amitriptyline (Elavil, Endep), or gabapentin (Neurontin), in addition to topical creams, are often required to help manage the pain. Lidocaine and/or capsaicin (Qutenza, Capzasin) are also occasionally used on the affected area; both are used after the blisters resolve for control of pain in postherpetic neuralgia.
