Confluent and reticulated papillomatosis is characterized by net-like hyperpigmentation (darker color) of the central chest and mid back. Its cause is unknown but may be related in some cases to Malassezia colonization. Progressive macular hypomelanosis presents with hypopigmented (pale) macules and patches on the trunk and may very much resemble tinea versicolor. In this case, however, the KOH is negative and there is no response to systemic or topical antifungal medications. Pityriasis alba are pale oval areas that appear on the face and are related to eczema and other sensitive skin conditions. This is more likely to respond to topical steroid treatment than antifungal medications. It would be unlikely for vitiligo (loss of color) to first present on the back and shoulders, but it is possible.

Also known as pityriasis versicolor, these spots will not tan in the sun, so you may not even notice them until you get a tan and the light spots stick out. The spots start out small, usually on the upper chest, upper back, shoulders and neck. They then grow slowly and develop into larger spots.

Sometimes the spots are so faint that people do not realize they have tinea versicolor. If tinea versicolor causes light spots on the skin, it can be mistaken for vitiligo. Vitiligo is a skin disease that causes the skin to lose its natural color.
August 23, 2018  Vol. 379  No. 8 Surgical Infections Create Account Patients need to realize that tinea versicolor is caused by a fungus that is normally present on the skin surface; thus, it is not considered a contagious disease. Sequelae from the disease are not permanent, and any pigmentary alterations resolve entirely 1-2 months after treatment is initiated. Treatment is needed to remedy the condition and for prophylaxis to prevent recurrences.

Selected specialties Hot, humid weather and lots of sweating can create a warm, moist environment for the yeast to overgrow. This is why the infection is more common in tropical countries. The yeast also likes an oily environment, so oily skin can play a part (and of course, teens and young adults can have oily skin).
Any specialist, including all primary-care providers (family practitioners, pediatricians, and internists), can competently treat tinea versicolor. Atypical cases and those not responding to usual therapy should be referred to dermatologist (a specialist in skin disorders and conditions) to consider alternative therapies and diagnoses.

    Tinea versicolor: Overview
Tinea versicolor is caused by a type of fungus that is found on almost everyone’s skin. It’s not clear exactly why the fungus grows and causes visible patches in some people but not in others. Tinea versicolor has nothing to do with poor hygiene, and it’s not contagious.

Tinea versicolor can usually be effectively treated using topical (externally applied) medications. These contain a substance that keeps the fungus from growing or kills it. Even with treatment, it can take several months for the skin to return to normal. Tinea versicolor is a skin condition that affects about 1% of the population in countries with a moderate climate. The main symptom is discolored (lighter or darker) patches of skin, mostly on the upper body. It is caused by a type of fungus that is found on most people’s skin. Tinea versicolor is not contagious.

But even if you get the yeast under control, your spots may still persist. That’s why early intervention is crucial if the spots on your back are really bothering you.

There are a few things you can do to prevent tinea versicolor. For starters, wear loose-fitting clothing: tight clothes create a breeding ground for yeast, so stick to breathable fabrics, says Dr. Katta. You should also shower immediately after your workout to prevent a sticky, warm space for yeast to grow.
Tinea versicolor is a common skin condition due to overgrowth of a skin surface yeast (type of fungus). This overgrowth results in uneven skin color and scaling that can be unsightly and sometimes itch. The yeast normally lives in the pores of everyone’s skin. It thrives in oily areas such as the neck, upper chest and back.
