Ghalayani et al., (2009) evaluated the level of IgA and IgG in patients with OLP and lichenoid reaction Lesions. The results showed higher level of IgA and IgG in patients than normal individuals in both groups. An increase in the salivary IgA level was observed in patients with oral leukoplakia, OLP and carcinoma of the oral cavity (Sato, 1991; Sistig et al., 2002). Also, in a study conducted with us, IgA level in patients with OLP was higher than healthy individuals, although this difference was not significant (Nosratzehi et al., 2014).

Within a month, it had spread in linear form between both my ankles; then it spread to the top of my right foot. My tongue had a white-patchy mucus-like coating on it that continued to spread to the inside walls of my mouth. I visited my family doctor. He had little clue and recommended me to a dermatologist. He instantly diagnosed it as lichen planus. It had also, by that time, spread to the inside wall of my labia. Needless to say, the itching was unbearable. He put me on a treatment program of prednisone, which immediately helped regardless of the sleepless side effects. I also was prescribed a cortisone cream that basically had little effect. Upon completion of this treatment (prednisone), (within a day) I was back to uncontrollable itching, and it had spread to the soles of my feet. Upon my third visit to the dermatologist, I was put back on the prednisone (feeling better and less itchy) and am waiting for light therapy. I am anxious to see some results and hoping I haven’t scarred myself for life from scratching too much. I certainly feel for anyone with lichen planus.

The true incidence of corticosteroid-induced hypothalamus-pituitary-adrenal suppression is unknown, but it is of concern with prolonged use.7 Often, evidence of hypothalamus-pituitary-adrenal axis suppression is found on the laboratory results, even when clinical symptoms are absent. Careful long-term follow-up of patients receiving topical corticosteroid therapy is highly recommended to detect potential complications.

The photosensitizing drug in PUVA is called psoralen. Both the psoralen and the UVA light must be administered within one hour of each other for a response to occur. These treatments are usually given in a physician’s office two to three times per week. Several weeks of PUVA is usually required before seeing significant results. The light exposure time is gradually increased during each subsequent treatment. Psoralens may be given orally as a pill or topically as a bath or lotion. After a short incubation period, the skin is exposed to a special wavelength of ultraviolet light called UVA. Patients using PUVA are generally sun sensitive and must avoid sun exposure for a period of time after PUVA. Common side effects with PUVA include burning, aging of the skin, increased brown spots called lentigines, and an increased risk of skin cancer, including melanoma. The relative increase in skin cancer risk with PUVA treatment is controversial. PUVA treatments need to be closely monitored by a physician and discontinued when a maximum number of treatments have been reached.

Eczema / dermatitis McKnight’s Senior Living Seborrheic dermatitis is a clinical diagnosis based on the location and appearance of lesions. In infants, it may present as thick white or yellow greasy scales on the scalp; it is usually benign and resolves spontaneously. In adolescents and adults, seborrheic dermatitis typically presents as flaky, greasy, erythematous patches on the scalp (Figure 1), nasolabial folds (Figure 2), ears, eyebrows (Figures 3 and 4), anterior chest, or upper back.3  The differential diagnosis is lengthy (Table 12,4), but the correct diagnosis can usually be made clinically by the characteristic distribution of lesions and varying course of the disease.5 If the diagnosis is uncertain, a biopsy demonstrating parakeratosis in the epidermis, plugged follicular ostia, and spongiosis can confirm the presence of seborrheic dermatitis. The diagnosis can be challenging in patients with darker skin, but the same principles apply.
