In vitiligo, biopsy will demonstrate in the center of a lesion the loss of melanocytes (pigment cells) with special stains of the epidermis. The border of a lesion may be noted visually to be inflamed (inflammatory vitiligo), and a notable inflammatory infiltrate of CD 4+ and CD 8+ T lymphocytes may be seen on biopsy of an “active” border,39 although the inflammation may be clinically absent.

Evidence has been accumulating to suggest that amino acids, rather than lipids, are critical for the appearance of the diseased state. In vitro, the amino acid asparagine stimulates the growth of the organism, while another amino acid, glycine, induces hyphal formation. In vivo, the amino acid levels have been shown to be increased in the uninvolved skin of patients with tinea versicolor in two separate studies.
Providers In tinea versicolor, small round or oval patches of skin appear at first, and later merge into larger patches with irregular shapes. They are especially common on the back, chest, neck and/or arms. They may occur on the face as well, particularly in children.

Progressive macular hypomelanosis presents with hypopigmented (pale) macules and patches on the trunk and may very much resemble tinea versicolor. In this case, however, the KOH is negative and there is no response to systemic or topical antifungal medications. Pityriasis alba are pale oval areas that appear on the face and are related to eczema and other sensitive skin conditions. This is more likely to respond to topical steroid treatment than antifungal medications. It would be unlikely for vitiligo (loss of color) to first present on the back and shoulders, but it is possible.

Tinea versicolor is a benign skin disease that causes scaly macules or papules on the skin. As the name implies (versi means several), the condition can lead to discoloration of the skin, with colors ranging from white to red to brown. The condition is not considered contagious because the causative fungal pathogen is a normal inhabitant of the skin. Treatment leads to cessation of scaling within a few days, but discoloration may last for weeks to months. If scale cannot be provoked and new lesions are not developing, then there is no need to repeat treatment and the patient can be reassured that ongoing infection in unlikely.

Currently, health researchers believe that rosacea patients have a genetically mediated reduction in the ability to dampen facial inflammation that is incited by environmental factors such as sunburn, demodicosis (Demodex folliculorum in the hair follicles), flushing, and certain medications. Rosacea tends to affect the “blush” areas of the face and is more common in people who flush easily. Additionally, a variety of triggers is known to cause rosacea to flare. Emotional factors (stress, fear, anxiety, embarrassment, etc.) may trigger blushing and aggravate rosacea. Changes in the weather, like strong winds, or a change in the humidity can cause a flare-up. Sun exposure and sun-damaged skin is associated with rosacea. Exercise, alcohol consumption, smoking, emotional upsets, and spicy food are other well-known triggers that may aggravate rosacea. Many patients may also notice flares around the holidays, particularly Christmas and New Year’s holidays.
