In cases of oral lichen planus affecting the gums with contact allergy to mercury, the lichen planus may resolve on replacing the fillings with composite material. If the lichen planus is not due to mercury allergy, removing amalgam fillings is very unlikely to result in cure.
Oral lichen planus is a non-infectious, inflammatory condition that affects the membranes inside the mouth, and it’s a variation of the skin condition, lichen planus. For some patients, the symptoms are mild, but for others, painful sores and red, inflamed tissues cause discomfort. No permanent cure exists for the condition, but certain medications and lifestyle changes can offer relief.

For this validation, we randomly selected three pairs of patients and controls from among the 24 age-matched OLP patients and healthy control subjects. Figure 2 demonstrates the results of immunoblotting analysis of the expression of these three proteins in the saliva of OLP patients, compared with healthy controls. SHARE OLP is classified into reticular, erosive, atrophic, and bullous types.9 The erosive and reticular forms are the most common types and present as papules and plaques with interlacing white keratotic lines (Wickham striae) with an erythematous border.10 The higher frequency of erosive OLP compared to the reticular and atrophic forms, as previously observed by Silverman,11 is probably the result of the symptomatic nature of this lesion, which often prompts an evaluation visit. The occurrence of erosive OLP confined to the gingival mucosa is characterized by the presence of diffuse erythematous areas that may or may not be interspersed with desquamative and ulcerated foci. The lesions may occur following the gingival outline, and hyperkeratotic radiating straie can be found at the periphery of the erosive region. When erosive OLP involves the attached gingival tissue, it is called desquamative gingivitis. The lesions of erosive OLP migrate over time and tend to be multifocal.12

  8. Lichen planus can appear in the mouth in several different patterns. The reticular pattern (see right) is commonly found on the cheeks as lacy web-like, white threads that are slightly raised. These lines are sometimes referred to as Wickham’s Striae. The name lichen comes from a plant that is often seen growing on rocks with its mossy, web-like appearance.

    Oral lichen planus and oral lichenoid contact stomatitis can appear clinically and histologically identical. Clinically, oral lichenoid contact dermatitis is most frequently caused by dental amalgams. Mercury and palladium are the two most frequently implicated metals to cause this type of reaction. On clinical examination, the physician may suspect oral lichenoid contact dermatitis over oral lichen planus based on the location of the areas involved. The most reliable clinical manner to separate the two is when oral lesions are juxtaposed to the dental amalgams and nowhere else in the mouth. This is most prevelant in oral lichenoid stomatitis.
