You are here Vancouver resident Marilee Sigal wishes she had done just that when she had a shingles outbreak three years ago. Just turned 55 at the time, she was sitting with guests in her back garden on an evening in August, 2013, when she felt a sharp pain high on the left side of her back. At first, she attributed it to fallout from a fierce childhood battle with scoliosis, a sideways curvature of the spine.
“He diagnosed me in seconds with classic shingles,” she relates, noting that the virus had lain dormant in her cervical nerve and attacked the right side of her head and face. She was immediately prescribed anti-viral medication, the first line of defense against shingles, to help reduce pain and complications and shorten the course of the disease. These medications should be started within 24 hours of feeling pain or burning, ideally before the blisters appear – underscoring the importance of seeking prompt medical attention when shingles symptoms first occur.

During chickenpox, the virus spreads in the bloodstream and infects collections of nerve cells (ganglia) of the spinal or cranial nerves. The virus remains in the ganglia in an inactive (dormant or latent) state. The virus may never cause symptoms again, or it may reactivate many years later. When it reactivates, the virus travels down the nerve fibers to the skin, where it creates painful sores resembling those of chickenpox. This outbreak of sores (shingles) almost always appears on a strip of the skin over the infected nerve fibers and only on one side of the body. This strip of skin, the area supplied by nerve fibers from a single spinal nerve region, is called a dermatome. Sores may also appear on the dermatomes next to the affected dermatome.

The disease most commonly affects middle-aged females. It is infrequently found in children, with a prevalence of about 0.03%, and reports of this are scarce in the literature. The erosive and atrophic forms of OLP are less common, yet they are more likely to cause symptoms. OLP is the target of much controversy, especially in relation to its potential for malignancy. Thus, it is important for clinicians to maintain a high index of suspicion for all intraoral lichenoid lesions. Periodic follow-up of all patients with OLP is recommended. In view of the above, the authors highlight a case of gingival erosive lichen planus affecting a 17-year-old adolescent without concomitant cutaneous lesions, with special emphasis on clinical and microscopic characteristics of the condition and management with retinoids and steroid therapy.

The attributes and attribute levels represented in the choice experiments realistically characterize all treatment options available for moderate and severe psoriasis. To our knowledge, our study is the first to explore and compare preferences for process and outcome attributes for all available psoriasis treatments. Clearly, our findings should be verified in larger and more diverse patient samples before generalizations can be made. Our study included only a subpopulation of patients with moderate to severe psoriasis treated in a German university hospital, and patient preferences may be different among individuals with mild psoriasis. We cannot imply that our findings are transferable to patients in other cultures and health systems. Furthermore, the effects of other disease-related factors, such as disease duration, previous treatment experience, and comorbidities, on patient preferences for psoriasis treatment need to be explored.
