There is no cure for seborrheic dermatitis but there are treatments that can help reduce the frequency and severity of flares. In other words, appropriate treatment, can help make it appear that patients have minor disease or no disease at all. However, periodic use of anti-fungal and anti-inflammatory agents may be needed to keep the condition under control.

In infants, it may present as thick white or yellow greasy scales on the scalp; it is usually benign and resolves spontaneously. In adolescents and adults, seborrheic dermatitis typically presents as flaky, greasy, erythematous patches on the scalp (Figure 1), nasolabial folds (Figure 2), ears, eyebrows (Figures 3 and 4), anterior chest, or upper back.3  The differential diagnosis is lengthy (Table 12,4), but the correct diagnosis can usually be made clinically by the characteristic distribution of lesions and varying course of the disease.5 If the diagnosis is uncertain, a biopsy demonstrating parakeratosis in the epidermis, plugged follicular ostia, and spongiosis can confirm the presence of seborrheic dermatitis. The diagnosis can be challenging in patients with darker skin, but the same principles apply.

Shop AAD Eczema Provider Finder Although some doctors warn their patients that any treatment for scalp seborrheic dermatitis may lose its effectiveness after three months of use, a study evaluating a zinc scalp treatment at six and 11 months showed no loss of effect.20 A study of the long-term use of ketoconazole shampoo for maintenance therapy also showed no degradation in effectiveness over time.21 What may appear to be a worsening of symptoms despite continued use of a treatment may actually represent the natural, varying course of the disease. Often, reassurance and maintenance of therapy are all that is needed, reserving changes in treatment for persistent worsening of symptoms. Patients with symptoms that do not respond to any of the therapies outlined above may benefit from systemic anti-inflammatories and should be referred to a dermatologist.

Epidermal Leveling Seborrheic dermatitis is associated with normal levels of Malassezia but an abnormal immune response. Helper T cells, phytohemagglutinin and concanavalin stimulation, and antibody titers are depressed compared with those of control subjects. Zani et al report that even with antifungal treatments, there was no reduction in levels of Malassezia. [23] The contribution of Malassezia species to seborrheic dermatitis may come from its lipase activity—releasing inflammatory free fatty acids—and from its ability to activate the alternative complement pathway. [24]
Anti-inflammatory medications, such as topical corticosteroids or calcineurin inhibitors, may also be used, especially if the disease is resistant to initial antifungal treatment.

Light therapy is also called phototherapy. There are several types of medical light therapies that include PUVA (an acronym for psoralen + UVA), UVB, and narrow-band UVB. These artificial light sources have been used for decades and generally are available in only certain physician’s offices. There are a few companies who may sell light boxes or light bulbs for prescribed home light therapy.
Guttate psoriasis may prove especially responsive to phototherapy. Therapies such as UVB and PUVA have low efficacy for the treatment of nail psoriasis because of the blockage of the UV radiation by the intervening nail plate, so that systemic therapy or intralesional steroids may be best for these. [43]  In 2017, the US Food and Drug Administration (FDA) approved the addition of moderate-to-severe fingernail psoriasis data to the adalimumab prescribing information, based on results from a phase 3, multicenter, randomized, double-blind, parallel-arm, placebo-controlled clinical trial. [44]

The condition does not leave any permanent scar or pigmentary changes, and any skin color alterations resolve within 1-2 months after treatment has been initiated. Recurrence is common, and prophylactic therapy may help reduce the high rate of recurrence.

The white or hypopigmented type of pityriasis versicolor is thought to be due to a chemical produced by malassezia that diffuses into the epidermis and impairs the function of the melanocytes.

They didn’t hurt, and they were only faintly visible. But then they started to spread down to my chest. And the tanner I got, the whiter they looked on my skin. Sometimes they would itch, but I was mostly annoyed with how they looked—and bathing suit season only made it worse.
